The Direct Service Provider will provide specialized support services to youth and their families, promoting independence, growth, and community integration. - Provide educational and advocacy resources to support the youth and their caregivers, promoting self-sufficiency and community engagement.
- Enhance the youth's integration into their family unit and community, leveraging resources and services to support their unique circumstances.
- Collaborate with caregivers and other service providers to ensure seamless support and referrals.
- Regularly update service plans, obtaining necessary consents and documents, and ensuring plans reflect the youth's evolving needs.
- Attend regular meetings with the supervisor and other service providers to ensure alignment with best practices and protocols.
- Report any incidents or concerns promptly to the supervisor.
- Document all interactions, services provided, and progress notes in a timely and accurate manner.
- Associate's degree or higher in a related field.
- Bilingual Spanish required.
- At least one year of experience in a related human services role.
- Knowledge and experience working with adolescents and special needs populations.
- Strong cognitive skills, professional demeanor, and comprehensive documentation skills.
