Regency Health & Rehabilitation Center in Yorktown, Virginia is seeking an experienced Director of Discharge Planning and Social Services to help residents and families prepare for their transition home. As a key member of our team, you will directly coordinate the patient's discharge, including supplies, transportation, and other services to ensure a safe transition. If you are passionate about helping people and delight in their recovery, we want to talk to you. We are looking for an energetic and dynamic self-starter with 2 or more years of experience in Discharge Planning and Social Services in a healthcare setting. The ideal candidate has a history of successful positions involving direct customer service, social services, coordination, and problem-solving, and teamwork. Strong communication and collaboration skills are a must. Your day-to-day responsibilities will include:
  • Researching, identifying, and arranging external services for residents such as Chaplaincy, Advocates, Responsible Parties, and optimal care environments.
  • Scheduling discharge meetings and coordinating home visits, home care, therapy according to resident needs.
  • Directing timely and efficient five-day discharge planning meetings for all newly admitted residents.
  • Assessing patient's support systems and home situation to assist in decision-making regarding discharge plan and needs.
  • Facilitating effective bed management by strategic evaluation and internal arrangement to maximize availability of transitional and extended care services.
  • Meeting with and assisting residents with social issues such as conflict resolution, problem-solving, and care coordination.
  • Conducting customer satisfaction surveys.
Qualified candidates will have a 4-year degree in Social Work, Psychology, Sociology, Education, Counseling, Criminal Justice, or other human services related degree and at least 1 year of Social Work experience in a healthcare setting.
