Care Coordinator

We are seeking a highly skilled Care Coordinator to join our team at Sanitas Medical Center. As a Care Coordinator, you will play a critical role in ensuring smooth patient flow throughout the day by scheduling/rescheduling patient appointments, answering phone calls promptly, and multitasking effectively.
Key Responsibilities- Engage and attract patients, solve problems, and work as a team with the rest of the staff to provide exceptional customer service.
- Orient and educate patients and their families by meeting them, explaining the role of the patient care coordinator, initiating the care plan, and providing educational information in conjunction with direct care providers.
- Monitor delivery of care by completing patient rounds, documenting care, identifying progress toward desired care outcomes, intervening to overcome deviations in the expected plan of care, and reviewing the care plan with patients in conjunction with the direct care providers.
- Respect patients by recognizing their rights and maintaining confidentiality.
- Review and analyze information in our EHR, develop interdisciplinary care plans, and other case management tools by participating in meetings, coordinating information and care requirements with other care providers, resolving issues that could affect smooth care progression, fostering peer support, and providing education to others regarding the case management process.
- Evaluate outcomes of care with the interdisciplinary team by measuring intervention effectiveness with the team and implementing team recommendations.
- Maintain quality service by establishing and enforcing organization standards, maintaining patient care databases, and ensuring the permanent update of Date Bases to improve patient care and treatments.
- Work closely with the Insurance Company to improve patient care and treatment, create and analyze statistical data, and liaise between the insurance company and Sanitas to improve patient flow through the system.
- Process daily cash collection and generate reports, and ensure the proper use of medical and office supplies.
- High school diploma or equivalent and graduation from a nationally accredited Medical Assistant (MA) program with active MA national certification.
- Experience in Microsoft Word, Excel, and PP.
- Ability to think on their feet to solve problems for patients while achieving care program team goals.
- Detail-oriented to ensure accuracy of reports and data.
- Ability to maintain effective and organized schedules to ensure timely patient flow.
- Ability to manage stressful and fast-paced work environments.
- Certified Clinical Health Coach (certification required to be obtained within the first 4 months of employment).
- CPR/BLS for Healthcare Providers is required.
- Two (2) years Medical Assistant experience in primary care setting is required.
