Job Title: Private Duty Nurse LVN: Adult Patients

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LVN to provide high-quality care to adult patients in their homes. As a Private Duty Nurse, you will work one-on-one with patients to develop and implement personalized care plans that meet their unique needs and goals.

Responsibilities:
  • Implement and coordinate care plans in collaboration with patients, families, and healthcare teams
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Document patient care and progress in a timely and accurate manner
Requirements:
  • Current, unrestricted LVN license in the state of practice
  • Current CPR certification
  • One year of experience in home health or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
