Job Summary

The successful candidate will provide one-on-one care to patients in their homes, assisting with daily living activities and administering medications as prescribed. Our nurses work closely with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.

Responsibilities
  • Implement and update patient care plans in collaboration with healthcare teams
  • Administer medications, treatments, and therapies as prescribed
  • Conduct patient assessments and coordinate care with healthcare providers
  • Provide education and support to patients and families on disease management and self-care techniques
  • Document patient care and progress in a timely and accurate manner
Requirements
  • Active, unencumbered license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • At least six months of recent experience as a Licensed Nurse in a clinical care setting
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
