Job Overview

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to provide high-quality care to adult patients in their homes. As a Private Duty Nurse, you will play a vital role in helping our patients transition from hospital to home and ensuring they receive the continuous care they need to thrive.

Responsibilities:

  • Implement and coordinate care plans in collaboration with physicians and other healthcare professionals.
  • Conduct thorough patient assessments and document care provided in a timely and accurate manner.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Provide education and training to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with the care team to ensure seamless care transitions and address any concerns or changes in patient condition.

Requirements:

  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification from American Red Cross, American Heart Association, or BLS.
  • One year of experience working as a licensed nurse, preferably in a clinical care setting.
  • Ability to travel and work in a variety of environments.
