We are seeking a skilled and compassionate Private Duty Nurse - LVN to join our team at Aveanna Healthcare. As a Private Duty Nurse, you will play a vital role in providing one-on-one care to our patients in the comfort of their own homes.
Key Responsibilities- Implement and manage a patient's prescribed plan of care, ensuring seamless coordination with healthcare providers.
- Conduct thorough patient assessments and develop personalized care plans to meet their unique needs.
- Administer medications, treatments, and therapies as prescribed by physicians, while maintaining accurate and timely documentation.
- Provide education and training to patients and their families on health promotion, disease prevention, and management.
- Collaborate with the care team to address patient concerns, changes in condition, and other issues that may impact care.
- Active, unencumbered license (RN/LPN/LVN) in the state of practice.
- Current CPR certification from a recognized organization.
- TB skin test (current within the last 12 months).
- At least six months of prior hands-on nursing experience (preferred but not required).
- Reliable transportation for patient visits.
