Advanced Practice Clinician

Key Responsibilities- Conduct in-home assessments to identify potential health risks and develop personalized care plans.
- Perform evidence-based practice screenings, including point-of-care testing, to inform diagnosis and treatment.
- Communicate findings to patients' primary care physicians to ensure seamless care coordination.
- Identify urgent and emergent situations and provide appropriate interventions.
- Educate patients on disease processes, medication management, and compliance strategies.
- Active NP license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions.
- NPs: Ability to work autonomously or without formal supervision, with approval from their licensing board if applicable.
- ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or the ability to obtain national certification and/or NP license in state of assignment by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
