About the Job

We are seeking an experienced Family Health Specialist to join our team at Reap Health Care in Antioch, CA. As a critical member of our healthcare team, you will be responsible for providing high-quality patient care and working closely with our medical staff to ensure the best possible outcomes.

Key Responsibilities:
  1. Conduct thorough physical examinations, diagnose and treat illnesses, and prescribe medications as needed.
  2. Develop and implement individualized treatment plans, considering patients' medical histories, lifestyles, and personal goals.
  3. Communicate effectively with patients and families, providing education and support throughout the care process.
  4. Work collaboratively with other healthcare professionals, including doctors, nurses, and therapists, to ensure comprehensive care.
  5. Stay current on the latest medical research and technologies, attending conferences and workshops as needed.

Requirements:

  • Master's degree in Nursing or related field.
  • Current license to practice as a Registered Nurse Practitioner in the State of California.
  • Board certification in Family Medicine or related specialty.
  • Ability to pass a background check and obtain any necessary certifications.

Compensation and Benefits:

  • $130,000 - $145,000 per year, depending on experience.
