Clinical Health Specialist

We are seeking a skilled Clinical Health Specialist to conduct health risk assessments on Medicare Advantage members. This ideal part-time job allows for flexibility and autonomy, enabling you to make up your schedule and earn supplemental income.
The Clinical Health Specialist will be responsible for conducting comprehensive in-home health risk assessments, identifying active and chronic disease conditions, and determining physical, mental, and social needs during the visit. The role also involves educating and counseling patients on identified conditions and screening procedures, as well as documenting findings and communicating recommendations using provided iPad.
Estimated Salary: $60-$80 per hour based on location and availability. Availability consisting of a minimum of five or more 7.5-hour days per month is required. Willingness to travel throughout your county and neighboring counties is necessary. Requirements include a Nurse Practitioner or Physician Assistant license in any specialty, excluding women's health and pediatrics.
