Facility Overview:

Our innovative rehabilitation center is equipped with the latest technology, ensuring that residents receive the best possible care during their recovery. We offer private accommodations for short-term residents seeking a comfortable and homelike atmosphere. Additionally, we provide private transportation services and employ a diverse team of multilingual nurses and rehabilitation therapists to ensure effective communication with all residents.

Qualifications:

  1. Preferred 1-2 years of experience in long-term care settings.
  2. Strong written and verbal communication skills.
  3. Good physical and mental health.
  4. Able to follow both oral and written instructions.
  5. Excellent organizational abilities.

Reporting Structure:

The Social Services Designee will report directly to the Director of Social Services.

Key Responsibilities:

  • Engage actively in assigned committees.
  • Oversee and coordinate discharge planning in accordance with relevant regulations.
  • Collaborate with healthcare organizations, residents and their families, and service providers to facilitate safe discharges.
  • Establish and maintain a transportation system to accommodate off-campus resident needs.
  • Educate residents, families, and staff regarding resident rights as per regulatory standards.
  • Conduct thorough psychosocial assessments following established policies.
  • Timely update care plans and progress notes.
  • Involve residents and families in care planning and encourage their participation in care conferences.
  • Attend all mandatory training sessions.
  • Support residents and families in adjusting to the transition of moving into a nursing facility.
  • Manage room assignments and maintain effective communication with residents and their families.
  • Plan discharges with active involvement from residents and families to ensure continuity of care.
  • Coordinate appointments for family discussions regarding health care decisions.
  • Arrange family conferences as needed.
  • Ensure compliance with advance directives through follow-up with residents and families.
  • Conduct annual reviews of residents' rights with residents and families.
  • Perform additional duties as assigned by the Director of Social Services or Administrator.
