About the Role:

The Haitian Centers Council Inc is seeking a highly skilled Senior Health Advocate to join our team. As a Senior Health Advocate, you will be responsible for providing ongoing case management and substance misuse support to members of the priority population. This includes conducting HIV/HCV testing and risk assessments, working with social media teams to develop marketing campaigns, and providing referrals for STI testing.

Responsibilities:

  • Conduct comprehensive assessments of clients' healthcare needs and provide guidance and support to address those needs effectively.
  • Assist clients in navigating the healthcare system, including scheduling appointments, obtaining referrals, and accessing necessary services.
  • Educate clients about their health conditions, treatment options, and available resources to empower them to make informed decisions about their healthcare.
  • C coordinate care among various healthcare providers and services to ensure seamless transitions and continuity of care for patients.
  • Connect clients with community resources, support services, and financial assistance programs to address social determinants of health and improve overall well-being.

Requirements:

  • Associate Degree or higher in a relevant field and equivalent practical experience required.
  • Minimum of three (3) years of similar work experience, facilitating groups and performing outreach/community engagement activities.
  • Certified in Rapid HIV/HCV Testing.
  • Previous experience delivering evidence-based interventions such as 3MV, RESPECT & D-Up.

What We Offer:

We offer a competitive salary of $63,000 - $75,000 per year, based on experience, and a range of benefits, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
