**Job Description:**

We are seeking a Patient Advocate to join our team at Northwell Health. As a key member of our healthcare staff, you will provide peer support, advocacy, and other recovery services to clients and caregivers.

**Key Responsibilities:**

  • Collaborate with the treatment team to facilitate implementation of the treatment plan.
  • Offer peer support services to participants prior to discharge and after their return to their communities.
  • Provide case management service to identified clients while maintaining the wellbeing of clients and integrity of the program.

**Requirements:**

  • Bachelor's degree in a related field or equivalent experience.
  • Experience working in a mental health setting preferred.

**Salary Range:** $70,000 - $90,000 per year.
