About the Position

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in patients' homes, reviewing their medical history, medication adherence, and social environment.

As a Home Risk Assessment Specialist, you will work collaboratively with our team of healthcare professionals to identify potential health risks and develop personalized care plans to address them. You will also provide education and support to patients and their families to help them manage their health effectively.

Responsibilities
  • Conduct comprehensive health assessments in patients' homes
  • Review medical history, medication adherence, and social environment
  • Develop personalized care plans to address identified health risks
  • Provide education and support to patients and their families
  • Collaborate with our team of healthcare professionals to ensure high-quality care
Requirements
  • Master's degree in Nursing or Physician Assistant studies
  • Current RN and NP or PA licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Excellent verbal and written communication skills
