Job Summary:
The Renal Dialysis Social Worker will work closely with the healthcare team to identify patients with barriers to discharge and develop effective plans to address these challenges. This will involve collaborating with patients, families, and healthcare providers to ensure seamless transitions of care and optimal patient outcomes.
Key Responsibilities:
- Conduct thorough assessments of patients' needs and develop individualized plans to address barriers to discharge.
- Collaborate with the interdisciplinary team to develop and implement effective discharge plans.
- Provide education and support to patients and families to promote self-advocacy and empowerment.
- Monitor patients' progress and make adjustments to plans as needed.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Master's degree in Social Work (MSW) or related field.
- Current licensure as a Licensed Certified Social Worker-Clinical (LCSW-C) or Licensed Professional Counselor (LCPC) in the State of Maryland.
- Minimum of 3 years of experience in social work or a related field, with a focus on complex care and renal dialysis.
