Job Title: Full-Time Hospice RN Case Manager

At American Veterans Hospice (AVH+), we are committed to providing compassionate and comprehensive hospice care to veterans and non-veteran seniors in Southern California. Our diverse team offers personalized care that honors the unique needs and experiences of each patient.

We are seeking a highly skilled and compassionate Full-Time Hospice RN Case Manager to join our care team. As a critical member of our interdisciplinary team, you will be responsible for delivering high-quality, patient-centered care to our hospice patients.

Key Responsibilities:

  • Manage and coordinate care for hospice patients, providing direct nursing care based on each patient's individual needs.
  • Conduct regular patient visits to assess physical, emotional, and spiritual needs, adjusting care plans as necessary.
  • Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and home health aides, to provide comprehensive care.
  • Educate patients and families on hospice care, symptom management, and the dying process.
  • Ensure timely documentation and compliance with all regulatory requirements.
  • Provide on-call coverage as part of the rotation to support patients and families outside of regular business hours.
  • Serve as a liaison between patients, families, and the healthcare team to ensure clear communication and continuity of care.
  • Maintain accurate and timely documentation in electronic health records.

Qualifications:

  • Active RN license in the state of California.
  • Minimum of 2 years of experience as an RN, with at least 2 year in hospice or palliative care.
  • Strong understanding of hospice philosophy and standards of care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Compassionate, patient-centered approach to care.
  • Valid driver's license and reliable transportation.
