Hospice RN Case Manager

We are seeking a highly skilled and compassionate Full-Time Hospice RN Case Manager to join our care team. As a critical member of our interdisciplinary team, you will be responsible for delivering high-quality, patient-centered care to our hospice patients.
Key Responsibilities:
- Manage and coordinate care for hospice patients, providing direct nursing care based on each patient's individual needs.
- Conduct regular patient visits to assess physical, emotional, and spiritual needs, adjusting care plans as necessary.
- Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and home health aides, to provide comprehensive care.
- Educate patients and families on hospice care, symptom management, and the dying process.
- Ensure timely documentation and compliance with all regulatory requirements.
- Provide on-call coverage as part of the rotation to support patients and families outside of regular business hours.
- Serve as a liaison between patients, families, and the healthcare team to ensure clear communication and continuity of care.
- Maintain accurate and timely documentation in electronic health records.
Qualifications:
- Active RN license in the state of California.
- Minimum of 2 years of experience as an RN, with at least 2 year in hospice or palliative care.
- Strong understanding of hospice philosophy and standards of care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of an interdisciplinary team.
- Compassionate, patient-centered approach to care.
- Valid driver's license and reliable transportation.
