RN Case Manager Hospice Professional

As a RN Case Manager, you will be responsible for planning, organizing, and directing hospice care, with a focus on community health education and experience.
Key Responsibilities:
* Completes initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
* Provides physical assessments and histories of current and previous illnesses
* Develops care plans that establish goals based on nursing diagnoses and incorporates palliative nursing actions
* Administers medications and treatments as prescribed by physicians
* Counsels and provides healthcare instructions to patients as appropriate
* Ensures arrangements for equipment and other necessary items and services are available
* Assists patients with activities of daily living and facilitates self-sufficiency and optional comfort care
Requirements:
* Current RN license to practice professional nursing in CA
* Graduate of National League for Nursing accredited school of nursing
* Current CPR certification
* Licensed driver with an automobile that is insured
* Minimum of one year of experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license
* Minimum of two years experience in public health, home care, or hospice nursing
* Demonstrates excellent observation, problem-solving, verbal, and written communication skills
* Ability to organize, prioritize workload independently, and document timely
