Job Summary

As a Licensed Vocational Nurse (LVN) at Continental Post Acute, you will play a vital role in providing high-quality care to individuals with developmental disabilities. We are seeking a dedicated and compassionate LVN to join our team and contribute to the well-being of our residents.

Key Responsibilities

• Provide direct care to residents, including assistance with daily living activities, such as bathing, toileting, dressing, and grooming.
• Implement individualized care plans, based on assessments and goals established by the care team.
• Administer medications and treatments, as authorized within the scope of the LVN's practice.
• Monitor residents' physical condition and behavior, reporting significant changes to the care team.
• Participate in interdisciplinary team meetings to develop and implement individual program plans.

Requirements

• Current LVN license in the state of California.
• Ability to work varied shifts, including mornings, afternoons, and nights.
• Strong communication and interpersonal skills.
• Ability to maintain confidentiality and handle sensitive information.
• Willingness to participate in ongoing training and professional development.
