Role Responsibilities

The Care Manager (RN) plays a crucial role in assisting members eligible for care coordination and case management services to achieve their highest level of health.

This position involves engaging with members and healthcare providers to evaluate, plan, and set personalized goals for each member. The Care Manager will facilitate and coordinate care while ensuring quality and the use of cost-effective resources.

As a primary point of contact, the Care Manager advocates for members within the care coordination program.

Key responsibilities include:

  • Assessing members to identify care coordination and case management needs.
  • Conducting comprehensive evaluations of environmental, psycho-social, and support requirements.
  • Identifying challenges and barriers to effective care coordination and implementing suitable interventions.
  • Developing individualized care plans aimed at addressing and resolving issues, enabling members to achieve optimal health outcomes.
  • Setting both short-term and long-term goals with defined timelines for completion, sharing these objectives with members and their families as appropriate.
  • Determining and executing the necessary level of intervention based on members' needs and clinical progress.
  • Scheduling follow-up communications as needed and making appropriate referrals.
  • Taking proactive measures to resolve member concerns.
  • Documenting progress towards goals and the resolution of issues.
  • Coordinating care and services with Account Managers, Complex RN Case Managers, and relevant healthcare providers.
