Role Summary
Under the guidance of a licensed pharmacist, this role is responsible for managing the daily operations of the medication management process and delivering comprehensive pharmaceutical services to patients ranging from infants to young adults. This position adheres to the regulations set forth by the state pharmacy board and the standards of the pharmacy department. The role reports to the Outpatient Pharmacy Supervisor and receives mentorship from Clinical Pharmacists and the Senior Pharmacist.
Key Responsibilities
- Inputs and/or confirms medication orders in the pharmacy management software system with precision and efficiency.
- Processes outpatient prescriptions accurately and provides both verbal and written counseling to patients and/or caregivers; ensures the suitability of all medication dosages based on the patient's diagnosis, weight, age, allergies, and potential drug interactions.
- Ensures compliance with departmental policies and procedures, timely processing of medication orders, and that the quality of dispensed products meets established departmental standards. Confirms the appropriateness of selected medications and guarantees that patients receive prescribed medications according to dose, route, formulation, regimen, and generic alternatives.
- Serves as a communication link between the pharmacy and medical/nursing staff regarding pharmacy services; identifies and addresses issues related to drug therapy as necessary (e.g., allergies, drug interactions, therapeutic duplications, adverse drug reactions).
- Initiates and manages medication occurrence reports and adverse drug reaction reports as required; engages in departmental data collection and reporting for Quality Assurance and Performance Improvement initiatives.
- Assists in the preparation of non-sterile medications following skills validation with a qualified observer.
- Participates in the Prescription Discharge Delivery Service and provides patient counseling under pharmacist supervision as appropriate.
- Completes Controlled Substance Documentation Records accurately when dispensing controlled substances.
- Engages in ongoing self-education activities while preparing for licensure examinations.
- Performs additional job-related tasks as assigned.
