Job Summary

Sutter Health is seeking a highly skilled Ambulatory Clinical Pharmacist III to join our team. As a key member of our clinical team, you will be responsible for delivering pharmaceutical care through the provision of patient-centered clinical services, medication information, education, medication preparation, and distribution. Your expertise will ensure safe, effective, and cost-efficient medication therapy for our patients.

Key Responsibilities
  • Deliver pharmaceutical care through patient-centered clinical services, medication information, education, and medication preparation and distribution.
  • Exercise sound judgment and developed clinical skills to provide input to and implement the patient's plan of care based on diagnosis in a timely manner.
  • Gain confidence and cooperation from patients, their families/support groups, and other healthcare providers through competent patient assessment, attentive monitoring, and care, and effective communication.
  • Adhere to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering optimal patient care.
Preferred Criteria
  • Experience as a specialty pharmacist in a hospital setting.
  • Board Certification (e.g., BCACP/BCPS/BCOP/CSP) - EPIC and Beacon experience.
  • ASHP-Post Graduate Year 1 ASHP General Pharmacy Residency.
Job Requirements
  • This position requires flexibility to orient and work at provider clinics and specialty pharmacies.
  • Clinical knowledge and therapy management for Chronic Inflammatory disease states/HIV/Oncology.
  • Maintain excellent written and verbal communication skills and strong critical thinking skills.
  • Excellent organizational skills, and ability to prioritize/followup.
  • Flexible; able to cope with rapid change; comfortable with an ambiguous environment.
  • Ability to troubleshoot problems efficiently and effectively.
  • Ability to provide comprehensive medication management including initial assessments, toxicity evaluations/symptom management, and reassessments.
  • Capability to set priorities amongst a series of competing clinical and operational priorities and to effectively coordinate and direct the work of others to meet specified deadlines.
  • Proficient computer experience and accurate data entry skills in MS Word and Excel.
Accountabilities
  • Prepares medications by reviewing and interpreting physician orders; and detecting therapeutic incompatibilities.
  • Dispenses medications by compounding, packaging, and labeling pharmaceuticals.
  • Controls medications by monitoring drug therapies; and advising interventions.
  • Ensures that routine and priority tasks are completed within established departmental time frames.
  • Mentors and fosters a constructive teaching environment that helps students, interns, and Pharmacy Technicians build confidence in their skills, knowledge, and abilities.
  • Completes pharmacy operational requirements by organizing and directing technicians' workflow; verifying their preparation and labeling of pharmaceuticals; and verifying order entries, charges, and inspections.
  • Provides pharmacological information by answering questions and requests of health care professionals; counseling patients on drug therapies.
Clerical
  • Performs general clerical tasks, such as answering phones; updating patient information; or retrieving, entering, and revising patient electronic health records (EHR).
  • Prepares and completes accurate documentation related to patients in medical and departmental records as required.
Safety
  • Participates in quality improvement processes, including updating and revision of safety and procedure manuals and interdisciplinary projects.
  • Maintains a clean, neat, and safe working environment by complying with policies, procedures, and regulations.
  • Protects patients and technicians by adhering to infection-control protocols.
Compliance
  • Maintains strictest confidence of all patient protected health information (PHI) and protects all PHI from accidental, intentional, or inappropriate disclosure.
  • Prepares, administers, and documents medications and treatments in accordance with state and federal regulations.
  • Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration by monitoring nursing unit inspections; maintaining records for controlled substances; removing outdated and damaged drugs from the pharmacy inventory; maintaining current registration; studying existing and new legislation; anticipating legislation; advising management on needed actions.
Education
  • Doctorate: Graduation from an accredited school of Pharmacy.
Certification & Licensure
  • PHARMD-Licensed Pharmacist.
