Job Description: As a Physical Therapist at JAG Physical Therapy, you will be responsible for assessing patients' strengths and weaknesses, devising effective treatment plans, and maintaining a supportive environment that promotes healing and functional independence. You will work closely with an interdisciplinary team to provide education and training to patients, and collaborate with healthcare professionals to ensure seamless care transitions.
Key Responsibilities:
- Collects and records patient care data, following format and guidelines of the patient care service, regulatory agencies, and third-party payers
- Prepares for evaluations, obtains patient history, and exhibits knowledge of patient diagnoses and unfamiliar diagnoses
- Selects and administers tests and measures, synthesizes evaluation results to formulate diagnosis, goals, prognosis, and plan of care
- Establishes treatment plans based on evaluation results, goals, and best practice, collaborating with the interdisciplinary team, patients, and patient support systems
- Provides education/training to patients and/or patient support systems, including functional training, such as ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription, and training, and community reintegration
Required Skills and Qualifications: To be successful in this role, you must have a Bachelor's or Master's degree in Physical Therapy, be licensed as a Physical Therapist in the state where you will be practicing, and have completed state-regulated CEU requirements. You should also possess excellent oral and written communication skills, strong clinical decision-making skills, and the ability to maintain a positive and professional demeanor at all times.
