Job Summary

We are seeking a highly skilled Crisis Discharge Planner II to join our team at Grand Lake Mental Health Center. As a key member of our crisis services team, you will be responsible for providing case management services to individuals and groups in various settings, with a focus on discharge planning and coordination.

Key Responsibilities
  • Provide case management services to individuals and groups in various settings, with a focus on discharge planning and coordination.
  • Collaborate with the Crisis Treatment Team to evaluate the appropriateness of discharge and develop discharge plans.
  • Assist persons served in arranging for health care, housing, medication, and other financial and educational resources.
  • Maintain ongoing census status of contract hospital services and the Crisis Units.
  • Participate as a member of a treatment team, with an aim of preventing recurrence of crisis.
  • Consult with individual and community organizations on the improvement of services for persons served.
  • Arrange for appropriate services after discharge from the Crisis units for mental health and co-occurring disorders.
  • Complete all agency documentation of services in a thorough, accurate, and timely manner.
Requirements
  • High School Diploma or equivalent.
  • 60 hours or equivalent of college credit hours in a mental health related field.
  • ODMHSAS Case Management II Certification.
  • Knowledge of the principles and practices of clinical social work, mental health, and co-occurring disorders.
  • Ability to work cooperatively and effectively with clients, staff, management, and other professional groups.
