Key Responsibilities- Case Management: Provide varying types of case management and/or rehabilitative services in various settings to individual, families, and groups.
- Crisis Intervention: Provide crisis intervention services, as assigned.
- Schedule and Coordinate: Schedule, coordinate, and facilitate wraparound/family teams.
- Assessments and Plans: Complete Strengths Needs Cultural Discovery (SNCD) assessments, Care Plans, and Updates, Functional Assessments/Crisis Plans/Safety Plans.
- Flex-Fund Requests: Complete flex-fund requests when necessary.
- Service Monitoring: Monitor and evaluate services provided to families. Contact families as needed to monitor progress and inquire about new needs.
- Family Team Meetings: Facilitate or co-facilitate Family Team meetings and may type the agenda.
- Family Team Lists: Schedule and coordinate Family Team meetings and ensure both formal and informal supports are included and part of the team.
- Referral Screening: Contact referred families and the referral source for screening.
- Service Coordination: Provide the appropriate staff personnel with new family team lists and approved referrals.
- Client Assessment: The Care Coordinator function will be initiated through a non-diagnostic interview with the client and his/her family to assess service needs.
- Service Development: The Care Coordinator will identify needed services by completing a strengths assessment and develop a care plan.
- Client Goals: The goals for each juvenile client as identified in the family team will be tailored to reflect a juvenile client's particular needs in order to realize his/her full potential.
- Service Coordination: The coordination of services between agencies will be an ongoing task for the Care Coordinator.
- Crisis Intervention: The Care Coordinator is expected to develop cooperative working relationships with all agencies serving juveniles from which a client might be expected to receive direct or indirect services as well as establishing and maintaining a therapeutic relationship with those juvenile clients and their families who are in need of support.
- Communication: The Care Coordinator is expected to possess skills necessary to provide immediate crisis intervention on a twenty-four (24) hour basis.
- Team Collaboration: The Care Coordinator will need to maintain close communication with the Integrated Team Manager and other clinical and administrative staff in order to assure the highest quality of services are delivered to the juvenile client and his/her family.
- Targeted Support: Provides targeted support services, a system of care that is child-centered with the needs of the child and family dictating the types and mix of services provided and to assist in preventing an out-of-home or out-of-school placement.
Bachelor's degree in Psychology, Social Work, Sociology, or Criminal Justice, or related field. Minimum of one-year work experience in the mental health field or related social service field, preferred. Current certification as a Behavioral Health Case Manager II.
