Crisis Discharge Planner II

We are seeking a highly skilled Crisis Discharge Planner II to join our team at Grand Lake Mental Health Center. As a key member of our crisis services team, you will be responsible for providing case management services to individuals and groups in various settings, with a focus on discharge planning and coordination.
Key Responsibilities- Provide case management services to individuals and groups at risk for placement in the community or other arrangements.
- Assist persons served in arranging for health care, housing, medication, and other financial and educational resources.
- Maintain ongoing census status of contract hospital services and the Crisis Units.
- Participate as a member of a treatment team to prevent recurrence of crisis.
- Consult with individual and community organizations on the improvement of services for persons served.
- Provide advocacy with schools, courts, and other social service agencies as needed.
- High School Diploma or equivalent.
- 60 hours or equivalent of college credit hours in a mental health-related field.
- ODMHSAS Case Management II expectation of core competencies in relation to individuals with co-occurring disorders.
- Ability to work cooperatively and effectively with clients, staff, management, and other professional groups.
- Ability to exercise mature judgment in dealing with people.
- Ability to present ideas clearly and accurately.
- Ability to read and comprehend the English Language.
- Ability to communicate effectively, both orally and in writing.
This position requires regular and predictable attendance, as well as the ability to work in a fast-paced environment with multiple priorities. The ideal candidate will be able to work independently and as part of a team, with a strong focus on customer service and quality of care.
