Job SummaryWe are seeking a highly skilled Care Coordinator to join our team at Grand Lake Mental Health Center. The successful candidate will provide case management and rehabilitative services to individuals, families, and groups, with a focus on supporting the needs of children and families.
Key Responsibilities- Provide case management and rehabilitative services to individuals, families, and groups, with a focus on supporting the needs of children and families.
- Develop and implement care plans that address the unique needs of each client, including crisis intervention services as needed.
- Coordinate services between agencies to ensure seamless support for clients, including establishing and maintaining therapeutic relationships with clients and their families.
- Provide targeted support services to prevent out-of-home or out-of-school placements, and to assist clients in achieving their full potential.
- Collaborate with the Integrated Team Manager and other clinical and administrative staff to ensure the highest quality of services are delivered to clients.
Bachelor's degree in Psychology, Social Work, Sociology, or Criminal Justice, or related field.
Minimum of one year of work experience in the mental health field or related social service field, preferred.
Current certification as a Behavioral Health Case Manager II.
