About the Role

We are seeking a skilled Licensed Master Social Worker to join our team at Enhabit Home Health & Hospice. As a key member of our interdisciplinary team, you will provide comprehensive, family-oriented services to individuals in the comfort of their own homes.

Responsibilities
  • Conduct thorough assessments of patients' and families' psychosocial, environmental, and financial needs.
  • Develop and implement personalized plans of care in collaboration with patients, families, and other caregivers.
  • Provide case management and support to patients and families as needed.
  • Maintain accurate and detailed documentation of patient interactions and progress.
  • Collaborate with healthcare professionals to ensure seamless care coordination.
Requirements
  • Must be a Licensed Master Social Worker (LMSW) in the state of employment.
  • Must possess a Master's degree from a school of social work accredited by the Council on Social Work Education.
  • Must have at least one year of social work experience in a healthcare setting.
  • Medicare home health or hospice experience is preferred.
