About the Role
As an LPN/LVN, you will play a critical role in ensuring the health and well-being of our residents, providing personalized care and support under the guidance of our Director, Health and Wellness. Your responsibilities will include:
  • - Assessing resident health status and developing tailored care plans
    - Providing direct patient care, including medication administration and vital sign monitoring
    - Collaborating with interdisciplinary teams to ensure seamless care coordination
    - Participating in quality improvement initiatives and maintaining a safe environment for our residents

Key Qualifications
- High school diploma or equivalent; LPN/LVN licensure required
- At least one year of experience in nursing or caregiving, preferably in assisted living, home health, or memory care
- Excellent communication and interpersonal skills, with ability to work effectively in a fast-paced environment
- Familiarity with Microsoft Word and Excel preferred
