Responsibilities

As a Patient Care Coordinator, you will be responsible for:

  • Building rapport with patients, providers, and staff to ensure seamless communication and care coordination
  • Conducting weekly meetings with assigned providers to discuss active patient lists and implement changes to accelerate care and improve outcomes
  • Assuring patient compliance by reviewing daily schedules and delegating changes/modifications as needed

In addition, you will collaborate with our medical team to coordinate patient imaging and MD appointments, provide advanced knowledge of Alliance Health System services and technology, and complete mastery of the role and responsibilities of a scheduler.

We estimate an annual salary of $63,000 - $73,000 based on industry standards and market conditions.
