Role Overview
We are seeking a skilled Healthcare Navigator Specialist to join our team at Providence St. Joseph Health.
About the Role:
The Healthcare Navigator Specialist plays a vital role in coordinating care for complex patients from pre-admission to immediate post-discharge. As a key member of our inter-disciplinary team, you will work collaboratively with healthcare professionals both within and outside our organization to ensure seamless patient care.
Key Responsibilities:
- Care Coordination: Develop and implement personalized care plans for patients, ensuring continuity of care and achievement of optimal clinical outcomes.
- Transition Planning: Coordinate transitions between different levels of care, ensuring smooth handovers and minimizing disruptions to patient care.
- Patient Education: Educate patients and their families on health management, medication adherence, and self-care strategies.
- Community Referrals: Identify and connect patients with community resources, services, and support networks.
- Clinical Decision-Making: Collaborate with healthcare teams to make informed decisions about patient care, balancing clinical needs with social determinants and resource availability.
Requirements:
To be successful in this role, you will need:
- A Washington Registered Nurse License.
- At least 3 years of acute care hospital or community-based clinical nursing experience.
- A Bachelor's Degree in Nursing or Associate's Degree in Nursing within 3 years of hire.
- Care Manager Certification.
- 1 year of Care management, discharge planning, or utilization management experience in the acute, sub-acute, home health setting or managed care environment.
