Care Dimensions is a non-profit hospice and palliative care organization dedicated to providing compassionate care to patients and their families. We are seeking a skilled Hospice Nurse to join our team.

About the Role

As a Hospice Nurse with Care Dimensions, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. You will provide holistic care, addressing the medical, emotional, social, and spiritual needs of our patients.

Key Responsibilities
  • Provide high-quality, patient-centered care to patients and their families
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans
  • Conduct thorough assessments and develop effective care plans that address the unique needs of each patient
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Participate in ongoing education and training to stay current on best practices in hospice and palliative care
