About the Role

We are seeking a dedicated and compassionate Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will play a vital role in providing exceptional care and services to our residents, ensuring their physical, emotional, and social needs are met.

Key Responsibilities
  • Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Notify management of changes in condition and recommend adjustments in the level of care and service.
  • Assist residents in life skills and other life-enriching activities, as indicated on their individual profile.
  • Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
  • Responsible for designated group of residents during the shift, ensuring their safety and well-being.
Requirements
  • Dedication to serving seniors with excellent customer service skills.
  • Positive attitude, flexibility, and ability to work in a team environment.
  • High School diploma/GED accepted and may be required per state regulations.
  • In states where appropriate, must maintain certifications.
  • Must be at least 18 years of age.
  • Previous experience working with seniors preferred.
