Job Title: Social Worker

Come and join Cape Cod Healthcare Inc. as a dedicated Social Worker to provide exceptional care to our patients and residents

We are seeking a highly skilled and compassionate individual with a Bachelor's degree in social work or a related field from an accredited college or university, licensed in Massachusetts, and at least one year of supervised experience in a healthcare setting working directly with the elderly or disabled.

Estimated Salary: $65,000 - $80,000 per year based on location and experience, depending on qualifications and background, with opportunities for advancement and professional growth within the organization.

Key Responsibilities:
  • Develop and implement comprehensive assessment plans to determine patient/resident goals.
  • Collaborate with multidisciplinary teams to create patient/resident directed care plans.
  • Conduct thorough discharge planning to ensure seamless transitions.
  • Identify and refer patients/residents to community resources and organizations.
  • Complete Social Service sections of the MDS within CMS mandated timeframes.
  • Assist patients/residents with Advanced Care planning.
  • Facilitate communication and coordination among care providers.
  • Ensure timely and accurate documentation of social services progress notes.

Requirements:
  • Bachelor's degree in social work or a related field.
  • Licensed in Massachusetts.
  • At least one year of supervised experience in a healthcare setting.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Compassion, empathy, and understanding of the needs of the elderly and disabled.
