Job Summary:

We are seeking a skilled and compassionate Professional Care Coordinator to join our team at Cape Cod Healthcare Inc. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality care for our patients.

About the Role:

The successful candidate will be responsible for conducting comprehensive assessments to determine patient goals, developing patient-directed plans of care, and referring patients to community resources as needed.

Key Responsibilities:

  • Collaborate with interdisciplinary teams to develop and implement patient care plans that address their unique needs and goals.
  • Develop discharge plans in collaboration with patients and their families to ensure a smooth transition to post-acute care settings.
  • Refer patients to community resources and organizations to support their ongoing care and well-being.
  • Complete social service documentation within CMS-mandated time frames to ensure accurate and timely reporting.
  • Assist patients with advance care planning to ensure their wishes are respected and carried out.
  • Coordinate and participate in care/discharge planning and initiate referrals for home care services to support patients in need.
  • Facilitate communication with post-discharge residents to ensure successful transitions and minimize readmissions.
  • Ensure timely completion of social service progress notes to maintain accurate records.
  • Participate in facility committees to stay informed about resident needs and advocate for their rights.
  • Advocate on behalf of patients as needed to ensure their voices are heard and their needs are met.

Requirements:

  • Bachelor's degree in social work or a related field from an accredited college or university.
  • Licensure in Massachusetts as a social worker.
  • One year of supervised social work experience in a healthcare setting working directly with the elderly or disabled.
  • Membership in good standing with the National Association of Social Workers and the Academy of Certified Social Workers, Inc. (preferred).
  • English language proficiency (reading, writing, speaking, and understanding).
  • Knowledge of community resources to support patient care.
  • Able to make independent decisions and contribute to a collaborative team environment.

Salary:

$65,000 - $85,000 per annum based on experience and qualifications.
