We are seeking a highly skilled and compassionate Patient Services Liaison to join our team at MJHS Hospice & Palliative Care. As a key member of our interdisciplinary team, you will play a vital role in improving the patient and family experience while on hospice service.
Key Responsibilities- Manage the coordination of supportive calls to patients and families at end of life, ensuring timely follow-up and communication with the care team.
- Participate in and contribute to the Interdisciplinary Team Process, fostering a collaborative environment that prioritizes patient-centered care.
- Display exceptional professionalism in all communication with patients, families, and colleagues, upholding the highest standards of empathy and respect.
- Report all patient experience program shift occurrences and complaints, driving quality improvement initiatives.
- High School Diploma or equivalent required.
- Prior experience working within a healthcare environment, with a preference for hospice experience and knowledge of hospice HIS requirements.
- Excellent verbal and written communication skills, with the ability to effectively communicate with diverse patient populations.
- Mandarin/Cantonese language proficiency required.
- Proficiency in various computer programs, including Microsoft Word, Excel, and PowerPoint.
