Position Overview

The Patient Services Coordinator II is integral to the effective delivery of healthcare services. This role acts as a bridge for patient care operations within the practice, ensuring high-quality patient experiences during their visits. The Coordinator is responsible for educating patients about their treatment plans and coordinating services with Primary Care Providers and outpatient diagnostic testing facilities. The Patient Services Coordinator II prioritizes tasks in accordance with the needs of patients, their families, and caregivers.

Key Responsibilities

1. Collaborates effectively with the Primary Care Provider to implement patients' care plans, engaging in daily meetings and monthly case discussions, scheduling appointments, and coordinating necessary information and ongoing care.

2. Works alongside other clinical personnel to address and resolve challenges affecting the progression of patient care.

3. Maintains and updates a dashboard with alerts and reminders to streamline daily responsibilities, including visit status updates, follow-up appointments, medical review authorizations, specialty appointment coordination, and billing error resolution.

4. Communicates with insurance providers for patient pre-certifications and consults with the Department/Practice Manager regarding patient needs or escalated concerns.

5. Provides front-end support as necessary, documenting telephone interactions for appropriate routing to Providers.

6. Acts as a liaison among Physicians, patients, and clinical staff to connect patients with relevant internal and external resources.

7. Suggests improvements in processes and services to enhance patient satisfaction and address concerns.

8. Offers thoughtful and informative guidance to patients as they navigate their healthcare options.

9. Continuously enhances knowledge of medical terminology and practices relevant to the hospital through ongoing education.

10. Performs additional duties as assigned.

Qualifications

Minimum of 4 years of experience in healthcare or customer service roles.
