As an Elderly Care Coordinator at EAC Network, you will play a vital role in the Expanded In-Home Services For the Elderly Program. This position focuses on delivering holistic services to homebound elderly individuals, ensuring they can maintain a secure care plan and continue residing in their own homes.

Key Responsibilities:
  • Perform in-home evaluations and collaborate with clients and their families to create personalized care plans.
  • Assess eligibility for benefit programs and facilitate necessary referrals.
  • Provide immediate crisis intervention services when emergencies arise.
  • Regularly monitor service delivery and the status of clients.
  • Accurately document client information in the database.
Qualifications:
  • Bachelor's degree in a relevant human services field.
  • Strong communication skills, particularly with the elderly population.
  • Knowledge of local services in Nassau County and the specific needs of the geriatric community.
  • Proficient in computer applications and possess a valid NYS Driver's License.
  • Must complete Case Management Certification within five months of employment.
