We are a leading provider of healthcare services to post-acute and long-term care facilities in the Portland Metro area.
Overview:
As a Senior Medical Provider, you will work closely with our team of dedicated clinicians to provide fast and reliable access to high-quality healthcare, revolutionizing the way healthcare is delivered in the Long-Term Care setting.
About the Role:
You will be responsible for rounding on new admissions, focusing on transition management, quality improvement, and reducing hospital readmissions at various facilities throughout the Portland Metro area.
Responsibilities:
- Plan, direct, and provide primary care and post-hospital medical care to facility residents.
- Collaborate and coordinate patient care with facility staff and Advanced Practice Providers.
- Participate in Quality Improvement initiatives.
- Timely completion of documentation.
- Some PA supervision may be required.
Benefits:
- $145,000 estimated annual salary.

Schedule:** Monday-Friday, 8a-5p with no on-call or after-hours requirements.
Qualifications:
- Current, unrestricted Oregon Medical License.
- Active DEA license.
- Board certified in Geriatrics, Family Medicine, or Internal Medicine.
