Nurse Practitioner or Physician Assistant Role in Senior Community Care

At Optum, we're creating something new in healthcare by bringing holistic approaches that address the physical, mental, and social needs of our patients. As a Nurse Practitioner or Physician Assistant, you'll join our integrated healthcare teams in Lane County, OR, and provide high-end medical service to our most vulnerable patient populations.

This role offers extensive training in our unique approach, focusing on quality patient outcomes rather than volume. With approximately 4-6 patient visits per day, you'll have time to invest in your patients' well-being and build meaningful relationships.

We're looking for individuals who can conduct comprehensive assessments, manage patients through acute medical situations, and ensure accurate documentation. Our ideal candidate will communicate effectively with the interdisciplinary care team and facilitate agreement on patient plans of care.

Key Responsibilities:

  • Conduct thorough patient evaluations and create personalized care plans.
  • Manage patients with complex medical conditions, including testing, diagnostics, and interventions.
  • Collaborate with the care team to develop and implement effective treatment plans.
  • Ensure timely and accurate documentation of patient information.
  • Foster strong relationships with patients, families, and caregivers.

Requirements:

  • Active and unrestricted Nurse Practitioner or Physician Assistant license in the state of practice.
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Board certification in a relevant area, such as family medicine or gerontology.
  • Ability to obtain DEA licensure/prescriptive authority, if applicable.
