About the Role:
We are seeking a highly skilled and compassionate Family Nurse Practitioner or Physician Assistant to join our team at LifeLong Medical Care. As a member of our multidisciplinary team, you will provide high-quality outpatient primary care to our patients in a community health clinic setting.
Responsibilities:
- Provide preventive and primary health services to patients, including medical history and physical examinations, assessments, and treatment plans.
- Act as an advocate for clients and consult with other health team members as needed.
- Maintain accurate and up-to-date client records and perform laboratory and screening tests as required.
- Develop and implement individualized care plans, provide education and counseling to patients and families, and recognize and respond to situations requiring immediate medical attention.
- Support and participate in triage functions, both by phone and in-person, and perform other duties as assigned.
Requirements:
- Current California NP or PA license.
- Training and experience in geriatrics, internal medicine, and/or family practice with board eligibility or certification.
- Interest in working with diverse patient populations and commitment to a community health philosophy of prevention and primary care.
- Experience with electronic health records and ability to work effectively in a team-oriented environment.
What We Offer:
- Competitive salary range of $123,000-$144,000 per year, pro-rated depending on years since licensed.
- Sign-on bonus and excellent benefits, including medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs.
