Primary Care Physician

We are seeking a dedicated Primary Care Physician to join our team at LifeLong Medical Care. As a Primary Care Physician, you will be responsible for providing high-quality, patient-centered care to our diverse patient population.
Responsibilities:
- Provide preventive, urgent care, and primary care health services
- Perform clinical assessments, order diagnostic evaluations, and plan care programs
- Provide verbal and written education and counseling to patients, families, and caregivers
- Maintain patient records for clinic reporting, billing, and quality improvement
- Work in a variety of settings, including health centers, skilled nursing facilities, and clients' homes
Requirements:
- Current California MD or DO license
- Training and experience in geriatrics, internal medicine, and/or family practice with board eligibility or certification
- Interest in working with a diverse patient population and commitment to a community health philosophy of prevention and primary care
- Experience with electronic health records
Preferred Qualifications:
- Community Health Center experience
- Experience as part of a team-based care model

We provide medical, dental, behavioral health, and school-based services at more than 15 primary care sites across Oakland, Berkeley, and West Contra Costa County.
Benefits:
- Competitive salary range: $197,000.00-$218,000.00 for MD/DO, $123,000-$144,000 for NP/PA
