Job Summary
The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to assigned populations. The ACN utilizes a collaborative approach to anticipate, integrate, and coordinate patient care plans to maximize the ability to remain in a community setting.
Key Responsibilities
- Identify at-risk patient populations who require care management services in collaboration with physicians.
- Serve as a patient/family advocate and act as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.
- Support the mission, values, and management of patient care services at WakeMed.
- Actively support and incorporate the ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
Requirements
- No experience required.
- Diploma in Nursing or Associate's Degree in Nursing required.
- Registered Nurse licensure required.
Work Schedule
Monday through Thursday, Friday.
Weekend and Call Requirements
No weekends or call requirements.
