Job Summary
The Ambulatory Care Nurse (ACN) is a critical role in our organization, responsible for providing nursing care management services to assigned populations. This position requires a collaborative approach to anticipate, integrate, and coordinate patient care plans, maximizing the ability to remain in a community setting.
Key Responsibilities
- Identify at-risk patient populations who require care management services in collaboration with physicians.
- Serve as a patient/family advocate and act as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.
- Support the mission, values, and management of patient care services, actively promoting the ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
Requirements
- No experience required.
- Diploma in Nursing or Associate's Degree in Nursing required.
- Registered Nurse licensure required.
Work Schedule
Monday to Friday, 8am-4:30pm. No weekends or call requirements.
