Community Health Nurse

We are seeking a dedicated and compassionate Community Health RN to join our team at WakeMed Health & Hospitals. As a leading provider of health services in Wake County, we are committed to improving the health and well-being of our community.
Job SummaryThe Community Health RN is responsible for providing total comprehensive nursing to patients and their families through the implementation of a plan of care. This plan is based on the patient's developmental and health care needs identified through assessment, physician orders, and the patient's physical, psychological, and socioeconomic status.
Responsibilities- Implement and evaluate patient care plans to ensure high-quality, patient-centered care.
- Collaborate with patients, families, and healthcare teams to provide education and support.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Support the mission, values, and management of patient care services at WakeMed.
To be successful in this role, you will need:
- Diploma or Associate's Degree in Nursing.
- No experience required, but a passion for community health and patient care is essential.
This is a Weekend Days position, working every weekend from 7am to 7pm.
