Job Summary

We are seeking a highly skilled Licensed Practical Nurse to join our team at the Institute for Comm Living. As a LPN, you will play a critical role in providing high-quality care and support to individuals with serious mental illness and substance abuse disorders.

Key Responsibilities
  • Assume case management responsibilities for assigned caseload of individuals, providing comprehensive care and support to help them achieve their goals and improve their overall well-being.
  • Work collaboratively with an interdisciplinary treatment team to develop and implement individualized treatment plans, ensuring that each person receives the care and support they need to succeed.
  • Provide nursing care and support to individuals in their homes, communities, and other settings, as needed.
  • Conduct regular assessments and evaluations to identify the needs and goals of each individual, and develop and implement plans to address these needs.
  • Collaborate with other healthcare professionals, including registered nurses, physicians, and social workers, to ensure that each individual receives comprehensive and coordinated care.
  • Document all interactions, assessments, and interventions in a timely and accurate manner, maintaining confidentiality and adhering to all relevant laws and regulations.
  • Participate in ongoing education and training to stay current on best practices and evidence-based treatments, and to enhance your skills and knowledge as a LPN.
  • Perform other related duties as assigned, in support of the Institute for Comm Living's mission and goals.
Requirements
  • Valid NYS license as a Licensed Practical Nurse and current NYS registration.
  • At least one year of experience providing services to individuals with serious mental illness.
  • Ability to work effectively in a fast-paced, dynamic environment, and to prioritize multiple tasks and responsibilities.
  • Strong communication and interpersonal skills, with the ability to work collaboratively with individuals, families, and other healthcare professionals.
  • Ability to maintain confidentiality and adhere to all relevant laws and regulations.
  • Basic understanding of the causes and processes of mental illness and substance abuse disorder.
  • Willingness to continually learn and apply knowledge, and to participate in ongoing education and training.
Working Conditions

This position requires the ability to work in a variety of settings, including homes, communities, and other settings, as needed. The LPN will be required to travel to these settings, and to work in a fast-paced, dynamic environment.
