POSITION OVERVIEW:
We are seeking a dedicated and professional Float Nurse, specifically a Licensed Practical Nurse (LPN), who is committed to delivering exceptional care to a diverse patient demographic, including those with special needs and their families across our various health center locations.
KEY RESPONSIBILITIES:
1. Organizes daily tasks for nursing personnel.
2. Collaborates with the Nursing Director to develop and maintain effective systems for daily medical and nursing functions.
3. Actively participates in the Patient-Centered Medical Home (PCMH) Care team, engaging in daily meetings and ensuring adherence to PCMH standards by all staff members.
