About the Role

We are seeking a highly skilled and compassionate CareCoach Connect Advanced Practice Clinician to join our team at UnitedHealth Group Inc. As a key member of our care delivery team, you will play a vital role in providing high-quality, patient-centered care to our Medicare-aged patients.

Key Responsibilities
  • Provide comprehensive, longitudinal care to patients in their homes and virtually, consistent with your licensure and certification.
  • Diagnose and treat both acute and chronic conditions, including diabetes, hypertension, kidney disease, and other chronic medical conditions.
  • Order and interpret diagnostic lab work, x-rays, and EKGs.
  • Prescribe pharmacological and non-pharmacological interventions for both acute and chronic conditions.
  • Perform procedures based on the patient's condition, such as placing IVs, wound debridement, joint injections, vaccinations, and more.
  • Collaborate with lead physicians, specialists, and other healthcare team members to develop a plan of care.
  • Determine the effectiveness of the treatment plan, evaluate patient response, and revise the plan of care as needed.
  • Communicate effectively with patients and families to ensure understanding of diagnosis, prognosis, and treatment plans.
  • Manage census of longitudinal care patients and work with the team to oversee their care on a regular basis.
  • Transition patients to hospice or back to usual care when clinically necessary.
  • Perform transition of care evaluations and acute visits for patients as needed.
  • Prepare and maintain accurate patient records and documents.
  • Provide education to patients and families on chronic disease management, symptom management, and quality of life expectations.
  • Participate in on-call responsibilities as required.

Requirements
  • Graduate of an accredited Nurse Practitioner or Physician's Assistant Program.
  • Current licensure through the State Board of Nurse Examiners or State Medical Board.
  • Current prescriptive authority from the State Board of Nursing or State Medical Board.
  • Comfortable with chronic condition management for Medicare-aged populations.
  • Valid driver's license within the state of work.
  • Ability to establish effective working relationships with patients and their families.
Preferred Qualifications
  • 2+ years of internal medicine/family practice experience with geriatric care.
  • Home health experience.
  • Knowledge of palliative care and end-of-life symptom management.
  • Bilingual (English/Spanish) language proficiency preferred.
  • Ability to assess the physical and psychosocial status of geriatric populations.
  • Ability to work independently and interdependently with the interdisciplinary team, showing initiative and motivation.
