Position Overview
We are excited to announce the opening of a new branch focused on Palliative Care. Join our team to make a significant impact in the lives of those facing serious health challenges.
As an Advanced Practice Nurse specializing in Palliative Care, you will play a crucial role in managing advanced illnesses and providing personalized care plans for our patients. You will work closely with our medical team to ensure that each patient receives tailored treatment, demonstrating exceptional clinical skills and ethical judgment.
Key Responsibilities:
- Collaborate with physicians and palliative care team members to deliver comprehensive advanced illness management and palliative care.
- Oversee patient care from initial assessment through to discharge, ensuring continuity and quality of care.
- Conduct thorough assessments, focusing on disease progression, functional changes, and symptom management.
- Develop individualized care plans in consultation with patients, families, and the healthcare team.
- Facilitate access to necessary services and resources as identified in the care plan.
- Participate in on-call services as required, providing responsive and competent care.
- Prescribe medications and order diagnostic tests in accordance with state regulations.
- Maintain regular communication with the medical team to discuss patient progress and modify care plans as needed.
- Document clinical findings and treatment plans accurately in the patient's medical record.
Qualifications:
Essential Skills:
- Strong leadership and communication abilities.
- Independent decision-making skills and self-motivation.
- Knowledge of palliative care principles and relevant regulations.
- Proficiency in electronic medical record systems.
Education and Experience:
- Master's degree in Nursing from an accredited institution.
- Minimum of two years of experience as a Nurse Practitioner.
- Preferred specialty in Family or Adult Medicine.
- Experience in value-based care practices is a plus.
Licenses and Certifications:
- Current RN license and Nurse Practitioner certification as required by state law.
- Collaborative agreement with a supervising physician.
- DEA license and any additional certifications relevant to palliative care.
