We are expanding our Palliative Care services and are seeking a dedicated Advanced Practice Nurse to join our team.

Our Advanced Practice Nurse in Palliative Care will oversee the management of complex health conditions, ensuring tailored treatment plans while exhibiting exceptional clinical expertise and ethical judgment.
Responsibilities include diagnosis, treatment, consultation, and follow-up under the guidance of the Palliative Medical Director or Primary Care physician.Key Responsibilities:
- Collaborate with the Professional Services Corporation (PSC) Physician, Primary Care Physician, and other members of the palliative care team to deliver comprehensive care through diagnosis, treatment, and follow-up for patients.
- Assume responsibility for managing patient care from initial assessment through to discharge.
Assessment should encompass:
changes in disease status, functional decline, nutritional and cognitive assessments, management of pain and distressing symptoms, and the overall impact of illness on the patient's and family's quality of life.
- Develop individualized care plans in consultation with patients, families, and the Advanced Illness team, including the Primary Care Provider.
- Facilitate access to necessary care when needs are identified in accordance with the care plan.
- Participate in on-call services as required, providing competent and responsive care.
- Prescribe medications, including controlled substances, as permitted by state law, and order necessary diagnostic tests and treatments.
- Communicate clinical findings to the PSC Physician regularly and reassess care plans to ensure effectiveness.
- Maintain effective working relationships with the supportive care team and involve them as needed to achieve optimal patient outcomes.
- Document clinical findings, treatment plans, and care provided in the patient's medical record in accordance with accepted standards.
- Stay informed about Palliative Medicine Quality Measures and ensure proper documentation and reporting.
Requirements for Advanced Practice Nurse:
Essential Skills:
- Strong leadership, interpersonal, and communication skills.
- Ability to make independent decisions and self-motivate.
- Understanding of Palliative Medicine principles, Medicare/Medicaid regulations, and interdisciplinary care processes.
- Compliance with continuing education requirements.
- Proficiency in electronic medical record systems.
Education and Experience:
- Master's degree in Nursing from an accredited institution or equivalent.
- At least two years of experience as a Nurse Practitioner.
- Preferred specialization in Family or Adult Medicine.
- Experience in Value-Based Care practices is advantageous.
- Familiarity with Part B billing processes.
- Ability to work independently in an Advanced Practice role.
Licenses and Certifications:
- Current RN license or equivalent as required by state law.
- Active Nurse Practitioner certification/license.
- Collaborative agreement with a supervising physician as required by state law.
- Part B Provider certification.
- Federal and state DEA licenses as required.
- ACHPN certification in Hospice and Palliative Care is preferred.
- Valid driver's license and automobile insurance.
