Job Summary

Boulder Community Health is seeking a highly skilled Behavioral Health Specialist to join our team. As a Behavioral Health Specialist, you will work closely with primary care providers to provide integrated clinical and psychosocial care to patients. Your expertise will help patients receive the behavioral support they need to improve their health.

Key Responsibilities
  • Provide short-term counseling and crisis intervention to patients in the primary care setting
  • Offer direct social work and case management services to patients and their families
  • Work collaboratively with providers and clinic care coordinators to ensure patients receive appropriate referrals to behavioral health specialists and community resources
  • Maintain a safe and comfortable environment for patients and families
  • Develop exemplary working relationships with patients, their families, and representatives
Requirements
  • Master of Social Work (MSW) degree
  • Licensed Clinical Social Worker (LCSW) in the State of Colorado
  • 1-2 years of supervised experience in an integrated primary care environment, acute care hospital setting, or human service agency
